1. Introduction
ChiroMEDIX is a chiropractic clinic in Chilliwack, British Columbia, led by Dr. Spencer Devenney, DC. We are committed to protecting the privacy and security of the personal information, including personal health information, that our patients and website visitors share with us. We collect, use, and disclose personal information in accordance with the federal Personal Information Protection and Electronic Documents Act (PIPEDA) and British Columbia's Personal Information Protection Act (PIPA).
2. What We Collect
From website visitors, we may collect the details you enter in our contact form, information you provide when requesting an appointment, and data gathered through cookies and analytics on our website, which is hosted on Wix. From patients, we collect the personal health information needed to provide safe, informed care, including your name, contact information, date of birth, health history, treatment records, and insurance and billing information.
3. Why We Collect It
We use your personal information to provide chiropractic care; book and confirm appointments; send appointment reminders, intake and consent links, and receipts; process payments and insurance claims; and meet our legal and regulatory college record-keeping duties.
4. Consent
We obtain your consent before collecting, using, or sharing your personal information. Consent may be express, such as when you sign an intake or consent form, or implied by the circumstances, such as when you book an appointment with us. You may withdraw your consent at any time, subject to legal or professional restrictions and reasonable notice, by contacting our Privacy Officer. Withdrawing consent may limit our ability to provide you with care. By giving us your email address or phone number, you consent to receive appointment reminders, confirmations, intake and consent links, receipts, and related text messages or emails, including as described in the Text Messaging (SMS) section below. You may opt out of non-essential messages at any time.
5. Use & Disclosure
We do not sell personal information. Mobile phone numbers and SMS consent are never shared with third parties for marketing purposes. We disclose personal information only to provide your care, process payments and insurance claims, work with trusted service providers who help run our clinic, or when required by law.
6. Service Providers
Some of our operations, including website hosting, email delivery, appointment scheduling, and payment processing, are carried out by trusted third-party service providers. These providers are contractually required to safeguard your personal information and may use it only to provide services to us.
7. Safeguards
We protect personal information using administrative, physical, and technical safeguards appropriate to its sensitivity. Electronic transmissions are encrypted, and access to patient records is limited to authorized staff who need it to do their jobs.
8. Retention
We retain health records in accordance with the requirements of the College of Complementary Health Professionals of British Columbia (CCHPBC). Records for adult patients are generally kept for at least 16 years from the date of the last entry. For patients who are minors, records are kept until the patient reaches the age of majority, plus the required retention period. Once the retention period ends, records are securely destroyed.
9. Your Rights
You have the right to request access to, or correction of, the personal information we hold about you. To make a request, please contact our Privacy Officer using the details below. We will respond within a reasonable time, in accordance with PIPEDA and BC PIPA.
10. Text Messaging (SMS)
Consent: We send text messages only to individuals who have given us their mobile number and agreed to be contacted by text, for example when booking an appointment or completing intake.
Types of messages: We send appointment reminders and confirmations, intake and consent links, receipts, and occasional service-related updates. These are transactional, care-related messages, not marketing messages.
Message frequency: Message frequency varies based on your appointments and interactions with our clinic.
Rates: Message and data rates may apply, depending on your mobile carrier and plan.
Opt-out and help: You can opt out at any time by replying STOP to any message. We will send one confirmation message and no further texts. Reply HELP for help, or contact us at admin@chiromedix.ca or (604) 792-9171.
No sharing of mobile data: Mobile phone numbers and SMS consent are never sold or shared with any third parties or affiliates for marketing or promotional purposes. Mobile information may be shared only with the service providers that help us deliver our messages, and only as needed to send them.
Carrier disclaimer: Message delivery is subject to your carrier and is not guaranteed. Carriers are not liable for delayed or undelivered messages.
11. Cookies & Analytics
Our website is hosted on Wix and uses cookies and similar technologies to help the site function properly and to gather analytics about how visitors use our site. You can control or disable cookies through your browser settings. Please note that some parts of our site may not work as intended if cookies are disabled.
12. Changes
We may update this Privacy Policy from time to time to reflect changes in our practices or the law. When we do, we will post the updated policy on this page with a new effective date.
